In order to process requests for Applications of Permit-to-Operate (PTO) and Certificate of Accreditation (CoA), please submit the following:
- Letter of Intent
- List of on-going projects (If applicable)
- Copy of amended or revised Articles of Incorporation or Articles of Partnership and By-laws. (If applicable)
- Contract with main locator, CJHDevCo (If applicable)
- Updated list of personnel
- Latest audited financial statements
- Certificate of Environmental Compliance (CEC)
- Sanitary and/or Health Certificate
- DOLE Clearance
- Copies of the following:
- Latest SSS contribution
- Latest Philhealth contribution
- Latest Pag-ibig contribution
- Clearance for Temporary Permit to Operate
- Other requirements that JHMC may deem necessary
